В современной онкологической практике все больший интерес вызывает выявление сторожевых лимфатических узлов (СЛУ) – первых узлов, стоящих на пути метастазирования злокачественных новообразований. Выявление СЛУ с последующим морфологическим исследованием позволяет персонализировать объем хирургического вмешательства при раннем раке молочной железы, меланоме, опухолях головы и шеи, новообразованиях шейки матки и эндометрия. В настоящее время идет активный поиск специфичных радиофармпрепаратов (РФП) для визуализации СЛУ. В рамках гранта федеральной целевой программы «Фарма-2020» был разработан оригинальный РФП на основе гамма оксида алюминия – [99mТс]-Al2O3. Были проведены доклинические исследования, которые продемонстрировали его эффективность и безопасность. Исследования фармакокинетики [99mТс]-Al2O3 показали, что через 24 часа после его подкожного введения около 12 % от введенной дозы аккумулируется в СЛУ, что позволяет адекватно производить их детекцию.</p></sec><sec><title>Цель</title><p>Цель. Изучить возможность применения [99mТс]-Al2O3 для визуализации СЛУ при раке молочной железы, гортани и гортаноглотки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Определение СЛУ было проведено у 55 больных раком молочной железы и 30 пациентов со злокачественными опухолями гортани и гортаноглотки. Выполнение исследования включало в себя проведение паратуморальной инъекции РФП, однофотонной эмиссионной компьютерной томографии с последующим качественным и количественным анализом полученных изображений и интраоперационной детекции лимфатических узлов с их последующим морфологическим исследованием.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Клинические исследования показали, что радионуклидная визуализация СЛУ с использованием [99mТс]-Al2O3 характеризуется высокой чувствительностью как при раке молочной железы, так и при раке гортани и гортаноглотки (94,5 и 90 % соответственно), за счет высокоинтенсивного накопления этого РФП в лимфатических узлах. Оптимальной временной точкой для проведения сцинтиграфического исследования после инъекции [99mТс]-Al2O3 является интервал 18–20 часов, что позволяет визуализировать максимально возможное количество лимфатических узлов с наиболее оптимальным уровнем радиоактивности для их детекции, как на томосцинтиграммах, так и интраоперационно.</p></sec><sec><title>Заключение</title><p>Заключение. При планировании оперативного этапа лечения для определения объема хирургического вмешательства в алгоритм обследования пациентов с опухолями молочной железы, гортани и гортаноглотки целесообразно включать радионуклидную диагностику СЛУ с применением в качестве радиофармпрепарата [99mТс]-Al2O3.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In modern oncology, the identification of sentinel lymph nodes (SLN), the first nodes that stand on the way of malignant tumor metastasis, is of increasing interest. Detection of SLN followed by morphological examination allows personalizing the surgical intervention for early breast cancer, melanoma, head and neck tumors, neoplasms of the cervix and endometrium. Currently, there is an active development of specific radiopharmaceuticals for SLN imaging. Within the framework of the grant from the Federal Target Program "Pharma-2020", an original radiopharmaceutical using gamma aluminum oxide – [99mТс]-Al2O3 was developed. Preclinical studies have been demonstrated its effectiveness and safety. Pharmacokinetic studies of [99mTc]-Al2O3 showed that 24 hours after its subcutaneous administration, about 12 % of the administered dose is accumulated in the SLN, which gives possibility for its detection.</p></sec><sec><title>Aim</title><p>Aim. To study the possibility of clinical [99mTc]-Al2O3 using for visualization of SLN in breast, larynx and laryngopharyngeal cancer.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The definition of SLN was carried out in 55 patients with breast cancer and 30 patients with malignant tumors of the larynx and laryngopharynx. The study included peritumoral radiopharmaceutical injection, single-photon emission computed tomography with qualitative and quantitative analysis of the images and radioguided surgery detection of lymph nodes with their subsequent morphological examination.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Clinical studies have shown that radionuclide imaging of SLN using [99mTc]-Al2O3 is characterized by high sensitivity in breast cancer, larynx and laryngopharyngeal cancer patients (94.5 and 90 %, respectively), due to the high-intensity accumulation of this radiopharmaceutical in the lymph nodes. The optimal time point for SPECT and radioguided examination is an interval of 18–20 hours after injection of [99mTc]-Al2O3, which allows visualizing the maximum possible number of lymph nodes with the most optimal level of radioactivity for their detection.</p></sec><sec><title>Conclusion</title><p>Conclusion.
