Доступ к инсулину ограничен глобальными барьерами, среди которых ключевым является монопольное ценообразование транснациональных компаний-оригинаторов. Утрата патентной защиты открывает путь для производства биоаналогов – более доступных препаратов, сопоставимых по качеству, эффективности и безопасности. Императивом разработки биоаналогов является проведение комплексных исследований сопоставимости с оригинальным препаратом, включающих физико-химическую и биологическую характеризацию. Разработка программы исследований сопоставимости, а именно выбор атрибутов и ортогональных методов для изучения структуры и функциональной состоятельности молекулы инсулина, должна основываться на глубоком понимании структуры и свойств молекулы, а также на последних научных знаниях.</p></sec><sec><title>Цель</title><p>Цель. Провести физико-химическую и биологическую характеризацию препарата РинЛиз® (GP40011) в сравнении с оригинальным препаратом Хумалог® (инсулин лизпро) для установления их сопоставимости в рамках оценки биоаналогичности.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Первичную структуру белка подтверждали с помощью времяпролетной масс-спектрометрии с матрично-активированной лазерной десорбцией/ионизацией и пептидного картирования. Структуры белка более высокого порядка были исследованы следующими методами: капиллярным изоэлектрофокусированием, методом Эллмана, круговым дихроизмом, флуоресцентной спектрофотометрией, динамическим рассеянием света. Сопоставимость профилей примесей исследуемых препаратов изучали с помощью методов эксклюзионной хроматографии и обращенно-фазовой высокоэффективной жидкостной хроматографии. Для характеризации функциональных свойств и биологической активности препаратов оценивали кинетику связывания с инсулиновыми рецепторами IR-A и IR-B, аутофосфорилирование инсулинового рецептора и проводили метаболические клеточные тесты («Захват глюкозы», «Инсулининдуцированный липогенез», «Ингибирование стимулированного липолиза»).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. По результатам оценки физико-химических свойств для препарата GP40011 была установлена высокая степень сходства с референтным препаратом Хумалог® по первичной структуре белка, структурам более высокого порядка, а также по профилю примесей. Оценка функциональных характеристик показала, что препараты GP40011 и Хумалог® имели идентичную кинетику связывания с инсулиновыми рецепторами IR-A и IR-B и демонстрировали сопоставимую биологическую активность.</p></sec><sec><title>Заключение</title><p>Заключение. По результатам проведенного исследования была продемонстрирована полная аналитическая сопоставимость препаратов РинЛиз® и Хумалог®. Проведенное исследование внесло значимый вклад в научное обоснование эквивалентности биосимиляра РинЛиз® и референтного препарата Хумалог®, тем самым обеспечив надежную основу для успешной регистрации и последующего клинического применения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Access to insulin is limited by global barriers, the most crucial of which is the pricing monopoly of multinational originator companies. The expiry of patent on the originator product opens the way for the production of biosimilars being affordable products with comparable quality, effectiveness and safety. The imperative for the development of biosimilars is to conduct comprehensive comparability studies with the originator product, including physicochemical and biological characterization. The development of a comparability study program, specifically the selection of attributes and orthogonal methods for analyzing the structure and the functional integrity of the insulin molecule, should be based on a deep understanding of the structure and properties of the molecule, as well as on the latest scientific knowledge.</p></sec><sec><title>Aim</title><p>Aim. To conduct physicochemical and biological characterization of the medicinal product RinLis® (GP40011) in comparison with the originator medicinal product Humalog® (insulin lispro) to establish their comparability within the framework of the biosimilarity assessment.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The primary protein structure was confirmed using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) and peptide mapping. Higher-order protein structures were investigated using capillary isoelectric focusing, Ellman's method, circular dichroism, fluorescence spectrophotometry and dynamic light scattering. The comparability of impurity profiles of the tested batches was studied using size-exclusion chromatography (SEC) and reversed-phase high-performance liquid chromatography (RP HPLC). Insulin receptor (IR-A and IR-B) binding kinetics assay, insulin receptor phosphorylation assay and metabolic cell-based assays (glucose uptake, insulin-induced lipogenesis, inhibition of stimulated lipolysis) were performed to characterize the functional properties and biological activity of the insulin products.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. GP40011 was found to have high similarity to the reference product Humalog® in terms of the primary protein structure, higher-order structures and impurity profile based on the results of the evaluation of the physicochemical properties. Functional characterization studies showed that GP40011 and Humalog® had identical insulin receptor binding kinetics to IR-A, IR-B and comparable biological activity.</p></sec><sec><title>Conclusion</title><p>Conclusion. The study results demonstrated full analytical similarity of RinLis® to Humalog®.
