Препарат Граник® (действующее вещество – производное индолинона GRS) – новый отечественный, независимый от оксида азота стимулятор растворимой гуанилатциклазы. Стимуляция растворимой гуанилатциклазы представляет собой перспективный подход к терапии сердечно-сосудистых заболеваний, вызванных тромбозом, особенно при противопоказаниях к назначению традиционных антиагрегантов и антикоагулянтов, создающих высокий риск кровотечений. Препарат Граник® находился на фазе I клинических исследований, и данная публикация посвящена оценке его безопасности и фармакокинетики у здоровых добровольцев.</p></sec><sec><title>Цель</title><p>Цель. Целью исследования являлось изучение переносимости, безопасности и фармакокинетики препарата Граник®, капсулы, 20 мг, у здоровых добровольцев после приема натощак в дозах 20, 40 и 60 мг в течение 7 дней.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В двойном слепом плацебоконтролируемом исследовании принимали участие 48 здоровых добровольцев мужского пола, получавших в течение 7 дней препарат Граник® в дозах 20, 40 и 60 мг или плацебо. Переносимость препарата оценивалась как хорошая, удовлетворительная или неудовлетворительная на основе субъективных данных от добровольцев. Оценка безопасности включала анализ нежелательных явлений, лабораторных показателей, данных электрокардиограммы и мониторинг жизненно важных функций. Концентрацию вещества GRS в плазме определяли методом высокоэффективной жидкостной хроматографии с тандемным масс-спектрометрическим детектированием. Статистический анализ выполняли с помощью языка программирования R v.3.5 (Revolution Analytics, США) и с применением методов параметрической и непараметрической статистики в зависимости от характера распределения данных.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Препарат Граник® в дозах 20, 40 и 60 мг после приема в течение 7 дней здоровыми добровольцами (по 16 участников в 3 когортах) продемонстрировал хорошую переносимость и безопасность. Фармакокинетические параметры значительно варьировали в зависимости от принятой дозы. Максимальная концентрация соединения GRS в плазме достигалась в среднем за 0,5–1,5 ч. При приеме препарата Граник® в дозе 60 мг вещество GRS длительно находилось в плазме: период полуэлиминации составлял 24,61 ± 10,40 ч. При приеме в дозах 20 и 40 мг удаление из плазмы ускорялось: период полуэлиминации уменьшался до 4,34 ± 3,61 ч и 3,69 ± 5,09 ч соответственно. Коэффициент накопления вещества GRS статистически значимо не различался при приеме препарата Граник® в диапазоне доз 20–60 мг в сутки.</p></sec><sec><title>Заключение</title><p>Заключение. При курсовом применении препарат Граник® продемонстрировал хорошую переносимость и благоприятный профиль безопасности. Полученные результаты обосновывают переход к следующей фазе клинических исследований препарата Граник®.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Granik® (the active substance is an indolinone derivative GRS) is a new domestic nitric oxide-independent stimulator of soluble guanylate cyclase (sGC). Stimulation of sGC represents a promising approach to the therapy of cardiovascular diseases caused by thrombosis, particularly in cases where traditional antiplatelet and anticoagulant agents are contraindicated and pose a high risk of bleeding. Granik® has been in Phase I clinical trials, and this publication is devoted to the evaluation of its safety and pharmacokinetics in healthy volunteers.</p></sec><sec><title>Aim</title><p>Aim. The aim of the study was to evaluate the tolerability, safety and pharmacokinetics of Granik® 20 mg capsules in healthy volunteers after administration under fasting conditions at doses of 20, 40, and 60 mg over a 7-day period.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A double-blind, placebo-controlled study involved 48 healthy male volunteers who received Granik® at doses of 20, 40, and 60 mg or placebo for 7 days. Tolerability was assessed as good, satisfactory, or unsatisfactory based on subjective data from the volunteers. Safety assessment was based on the analysis of adverse events, laboratory values, ECG data, and vital signs monitoring. GRS plasma concentrations were determined by high-performance liquid chromatography with tandem mass spectrometric detection. Statistical analysis was performed using the R v.3.5 program (Revolution Analytics, USA) and parametric and nonparametric statistics depending on the nature of the data distribution.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Granik® at doses of 20, 40 and 60 mg demonstrated good tolerability and safety after 7-day administration to healthy volunteers (16 participants in 3 cohorts). Pharmacokinetic parameters varied significantly depending on the dose. The maximum plasma concentration (Cmax) of the GRS compound was reached in 0.5–1.5 h. When Granik® was administered at a dose of 60 mg, the GRS substance remained in plasma for a long time: the half-elimination period (t1/2) was 24.61 ± 10.40 h. When Granik® was administered at doses of 20 and 40 mg, elimination from plasma was accelerated: the half-elimination period decreased to 4.34 ± 3.61 h and 3.69 ± 5.09 h, respectively. The accumulation coefficient of the GRS substance did not statistically significantly differ when Granik® was administered in the dose range of 20–60 mg per day.</p></sec><sec><title>Conclusion</title><p>Conclusion. Granik® demonstrated good tolerability and a favorable safety profile when administered as a course.
